In light of the recent Coronavirus/COVID19 outbreak, and in an effort to do everything I can to protect myself, my clients, and the community, I ask that you please carefully read and understand the below Terms & Conditions, then complete the following form and wellness check prior to your appointment.
TERMS & CONDITIONS OF SERVICE:
PLEASE READ TERMS AND CONDITIONS OF SERVICE CAREFULLY AS COMMENCEMENT OF SERVICE BY DIANNE MURPHY MAKEUP ARTIST & HAIR STYLIST (OR ANY OTHER CONTRACTED ARTISTS) IS CONFIRMATION THAT YOU ACCEPT AND FULLY UNDERSTAND ALL OF THE TERMS & CONDITIONS OUTLINED BELOW:
1.0 COVID-19 Terms and Conditions of Service
1.1 For the purpose of these Terms and Conditions of Service, we, us, our, artist/s and stylist/s refer to Dianne Murphy Makeup Artist & Hair Stylist and our contracted stylists who are providing either hair and/or makeup services. Client, you and your refers to the person named in the form below.
1.2 Once service commences by Dianne Murphy Makeup Artist & Hair Stylist, this form constitutes a binding agreement between the client (you) and Dianne Murphy Makeup Artist & Hair Stylist and/or our contracted stylist.
1.3 Dianne Murphy Makeup Artist & Hair Stylist reserves the right to alter these Terms and Conditions of Service and will notify you, the client, of any changes in writing.
2.0 CLIENT SERVICE CONDITIONS
2.1 The client MUST wash and/or sanitise hands with provided hand sanitiser before engaging in the artists working area.
2.2 The client MUST not bring drinks, phones or any other item/s within the artist/s 4m² working area unless agreed by the artist/s prior.
2.3 The client MUST NOT touch the artist/s, their tools, items or equipment under any circumstance unless directed by the artist/s.
2.4 The client MUST NOT touch or physically interact with any other person/s other than Dianne Murphy Makeup Artist & Hair Stylist artist/s while they are being serviced within the artists 4m² working area.
2.5 The client MUST disclose to Dianne Murphy Makeup Artist & Hair Stylist artist/s if presented with any flu like symptoms, sore throat, cough, fever or shortness of breath within the last 48 hours.
2.6 The client MUST disclose to Dianne Murphy Makeup Artist & Hair Stylist artist/s of any travel taken outside New South Wales within the last 14 days.
2.7 The client MUST disclose to Dianne Murphy Makeup Artist & Hair Stylist artist/s if been in contact with anyone who has tested positive to COVID-19.
2.8 Dianne Murphy Makeup Artist & Hair Stylist reserves the right to refuse service to any and all clients who refuse to adhere to the above mandatory conditions of service. A refund is not applicable under any circumstance if service is refused due to breach of the conditions of service or Dianne Murphy Makeup Artist & Hair Stylist/s feels at risk of infection.
3.0 ARTIST/S WORKING AREA/CONDITIONS
3.1 Clients, any of their friends, family or guests whether that person is part of the service or not, MUST not come within 1.5 meters of Dianne Murphy Makeup Artist & Hair Stylist/s and the client they are servicing. A 4m² area MUST be maintained from Dianne Murphy Makeup Artist & Hair Stylist/s and the clients they are servicing AT ALL TIMES. Dianne Murphy Makeup Artist & Hair Stylist reserves the right to terminate the service at any time if these social distancing parameters are breached and/or if the artist/s feels at risk of infection due to unacceptable social distancing behaviour.
3.2 There cannot be any more than 10 people (including Dianne Murphy Makeup Artist & Hair Stylist/s) in the area at a time and a social distanced area of at least 4m² per artist MUST be available. If the working area is only big enough for Dianne Murphy Makeup Artist & Hair Stylist/s and the serviced client/s, all other people must be removed from the room. If a suitable working area is not provided and or there are too many people within the room, Dianne Murphy Makeup Artist & Hair Stylist reserves the right to terminate the booking at any time if these government legislated parameters are breached and/or if the artist/s feels at risk of infection due to unacceptable room capacity breaches.
4.0 COVID-19 WAIVER/LIABILITY
When choosing to commence hair or makeup services by Dianne Murphy Makeup Artist & Hair Stylist, you are doing so at your own risk. By agreeing to the terms and conditions of service, you agree that Dianne Murphy Makeup Artist & Hair Stylist cannot be held accountable for any infection or contamination relating to infectious disease either directly or indirectly from being serviced by us. This includes but is not limited to
*Accidental injury by contamination of infectious disease to persons including but not limited to, injury resulting from the use of tools and equipment required for the service, injury to the client, any of their friends, family or guests whether that person is part of the booking or not, and any other person at the location that the booking is taking place at.
*Any other illness or infection incurred by circumstances outside of Dianne Murphy Makeup Artist & Hair Stylist’s control. It is the clients responsibility to disclose and inform the stylist/s if you have presented with any flu like symptoms, sore throat, cough, fever or shortness of breath in the last 48 hours or of any potential illnesses which could affect the health of the artist/s and or contaminate their equipment.
Dianne Murphy Makeup Artist & Hair Stylist/s will take all possible precautions and uphold a strict and stringent hygiene procedure as per the State Government required COVID-19 Safe Plan (See below the following form) to avoid and limit contamination and spread of infection. While all care is taken, no responsibility is accepted.
